 BIRTH RECORDS

 Birth Records are not public information. Copies of birth certificates are only available to the person named on the certificate (18 years or older), the person's parents, a lawful representative, or anyone who presents a court order. If you were born within the village limits of Johnson City, N.Y., and are over 18 years of age, you may obtain a certified copy of your birth certificate from our office.

 DEATH RECORDS

 Death Records are considered confidential by New York State Law and are available to the spouse, parent, or adult child of the decedent; someone who can demonstrate a legal claim; Veterans Administration Hospitals; federal agencies for official purposes; New York State agencies for official purposes; and someone who can demonstrate an official medical need. If the decedent died in the Village of Johnson City, N.Y., the record is on file in our office.

 You may obtain a copy of Birth Records and/or Death Records either in person or by mail.

 HOW TO OBTAIN RECORDS

  In Person

  Bring the completed application, photo identification of yourself (see forms of ID accepted below), and the $10 fee (cash, certified check, money order) for each record. The record can be obtained immediately.

  By Mail - Send the Following:

 - The completed application

 - A photocopy of one of the following forms of identification: Driver's License, Non-driver's license, Passport, Naturalization Papers, Military ID, Employer's Photo ID, Two utility bills showing applicant's name and address, Police report of lost or stolen ID (records will not be issued unless one of the above types of identification is enclosed)

 - Remit the required fee of $10 per certificate in the form of a money order or certified check only, made payable to "The Village of Johnson City, NY". (Sorry, we do not accept personal checks - do Not send cash or stamps)

 - Mail to: Office of the Village Clerk, 243 Main Street, Johnson City, N.Y. 13790. Requests are usually answered the same day in which it is received.
